T. I. Alekseenko ASSESSMENT OF THE FUNCTIONAL STATE OF CARDIO-RESPIRATORY SYSTEM IN TEENAGERS UNDER CONDITIONS OF A MEGAPOLIS

A priority objective of the social-economic development in the Russian Federation is overall protection and strengthening of children’s health. The human health is formed during ontogenesis so that every stage of which is characterized by a different level of the body development. Consideration of health as an integral state of the organism presupposes determination of its psychophysiologic, morphofunctional, typological and other indices that depend on individual regularities of the human development. Correlation of health and social factors, hereditary and genetic consequences of environmental pollution were studied.

The results of complex assessment of health status in teenagers residing in Moscow prove that during the last 10 years the number of healthy teenagers has decreased up to 8,6%. At present time the number of schoolchildren with functional dysfunction is 36,0%, suffering from chronic pathology with various degrees of severity - 55,4%. During the same period growth of functional disorders has been also revealed. For the increase of functional disorders accounts mainly poor blood circulation as well as chronic upper respiratory tract diseases, endocrine, metabolic and allergic disorders. On the whole occurrence of the above mentioned disorders of somatic health has grown on 47,3%. It creates negative background for development in the further age periods of health significant disorders.

Transition between health and disease is a complex theoretical and practical problem of the medical science. Till the present time it has been solved in the framework of nosologic approach: one class of healthy people and numerous classes of diseases to be considered. During the last ten years another view on this problem has been developed. During the prenosological approach numerous levels of health and as an independent class – disease is considered.

In IBMP RAS RF a program of prenosological examination of the population “Health Navigator” has been developed. With the help of this program 960 practically healthy Moscow schoolchildren of at the age of 14-17 were examined. The objective of our investigation was to determine the functional state of cardio-respiratory system in the teenagers from a large industrial center. The analysis of the obtained data showed that all the characteristics (heart rate at rest, systolic and diastolic pressure at rest, stroke volume at rest and with loading, blood minute volume with loading, CVS adaptability, respiration rate, lung vital capacity, oxygen maximum intake) are within the limits of the age norms. The tendency of shifting of values of each parameter to the side of lower limits is observed both in girls and boys. During loading tests of moderate power (Shtange’s hypoxic test, Rufie test, two-stage step-test) tension of cardio-respiratory system work was observed that was manifested in the decrease of CVS adaptability, oxygen maximum intake, general work capability, blood minute volume.

The developed technology enables to answer the most important questions: what part of children’s population has high, medium and low reserves of physical health, how effectively sanitary-prophylactic programs are implemented; how geographical, economic and social factors influence children’s health.
