COMPLEX TREATMENT OF AGE-RELATED MACULAR DEGENERATION

In the opinion of many authors there is no effective treatment of ARMD. This has determined the purpose and tasks of our research.

Objective: to develop and apply a new physiotherapeutic method to increase efficiency of ARMD treatment.

Materials and methods of treatment: for a complex ARMD treatment for the first time an ultrasound pneumomassage was performed by the local apparatus «ABMO». With the help of the control block the impulses of rarefaction (hypobaric regime) were transmitted on the eyeball and the tissues of the eye-socket with the infrasound power of 170 dB and frequency 4 hertz.

684 patients (1 238 eyes) were examined and treated. The medical treatment included vasodilating, antioxidant, vitamin-containing, antisclerotic, lipotropic drugs.

The main group of 70 patients (118 eyes) was treated with the above mentioned method to which infrasound pneumomassage was added.

Methods of clinical examination: visometry, sphereperimetry, biomicroscopy, tonometry, tonography, direct and indirect ophtalmoscopy, biomicroophtalmoscopy.

Results: Improvement in acuity after the treatment was noted, in the main group with a low visual acuity (<0,02) averagely from 0,01 ± 0,001 to 0,02 ± 0,001. In the control group the improvement rated from 0,01 ± 0,005 to 0,01 ± 0,007 after the treatment. In the patients with higher initial indices (visual acuity 0,5 – 0,7) the acuity averagely increased from 0,59 ± 0,02 to 0,7 ± 0,02 (in the control group 0,60 ± 0,01 to 0,67 ± 0,02). A positive dynamics was revealed after the complex therapy in conjunction with infrasound pneumomassage in average 46%, in the control group a positive dynamics was observed in 29,7% cases.

Dynamics of perimetrical parameters after the therapy in conjunction with the pneumomassage: The fields of vision were increase in 67,8% cases (in the control group in 26,2%). The changes in the amount of scotomas were not detected.

The induces of intraocular pressure before and after the complex therapy in conjunction with the infrasound pneumomassage: decrease of ICP was reported in 68% cases averagely by 2 mm Hg.

There were no complications after the complex conservative treatment of the retina in conjunction with the infrasound pneumomassage, which speaks about a high safety level of this method.

Conclusion: the conducted study has produced strong evidence enabling to recommend the method of infrasound pneumomassage for the complex treatment of aged-related macular degeneration.
