SPINAL TRAUMA AND FUNCTIONAL STATE OF A SPINAL CORD’S
AFFERENT WAYS |

What occurs to conducting ways of a spinal cord at a spinal trauma? It is possible,
that in a number of cases there can be an infringement of realization of excitation.
But how quantitatively to estimate these infringements? The functional status
of pyramidal ways can be estimated through a method of magnetic stimulation
(MS). In Altai region our colleagues have offered methods which to estimate
a functional state of afferent ways of spinal cord (Remnev A.G., Smirnov K.V.
Patent RU 2136328, in Russian; Remnev A.G., Kulikov V.P., Patent RU 2122827,
in Russian; Remnev A.G., Patent RU 2163460, in Russian). These methods were
carried out by magnetic stimulation. MS was performed in two points (cervical
and lumbar areas). Evoked potentials were registered from muscles of orbicular
muscles of eye. These diagnostic methods have passed a clinical expert estimation
and they are applied in the Russian clinics with success during several years.
The functional state of afferent ways was characterized as speed of excitation’s
distribution (SED). 84 healthy subject aged between 19 and 40 years and 29 subjects
with spinal trauma aged between 18 and 41 years were observed. The spinal trauma
was at a pectoral level and was qualified as a bruise of a spinal cord.
We investigated the patients 10-16 weeks after a spinal trauma. The patients
moved independently in this period. The results of the conducted research have
shown that SED on afferent ways in healthy subjects was 90,0±12,3±1,6
m/s (M±σ±m) and 37,4±10,7±2,1 m/s (M±σ±m)
in patients with spinal trauma. SED at healthy patients was considerably in
2-3 times higher than at the spinal trauma’s patients (p<0.01). Thus,
the carried out researches show an opportunity of definition of a functional
status of conducting ways of a spinal cord. It is necessary to note, that in
the patients with a spinal trauma even after the beginning of active own movements
infringement of function of conducting ways of a spinal cord - pyramidal ways
and afferent ways of the spinal cord can be observed. The offered diagnostic
approach can be applied in dynamic supervision over the patients at a spinal
trauma. This method of diagnostics can be applied in dynamic supervision of
the patients with spinal trauma.
