About 90 % of the population suffers from periodic pains in a lumbar region. Therefore diagnostics and treatment of pathology of lumbar department is one of the major problems of a modern medical science. The purpose of researches – to develop new, harmless, exact and inexpensive ways of diagnostics of a lumbar pathology. We have developed some new ways of diagnostics of a pathology of lumbar department with the help medical imaging technique - medical sonography (ultrasonography).
Actually the history of application of a method of ultrasonic diagnostics in diagnostics of a lumbar department pathology begins since 1978, when Porter R.W., Wicks M. and Ottewell D. have suggested to define the sizes of the vertebral channel by means of ultrasonic research (Porter R.W., Wicks М, Ottewell D., 1978). In 1984, Luis A. Portela has informed on possibility of application of forward access for diagnostics of hernias intervertebral disks. Further the considerable quantity of works and the researches devoted to this question has been spent. In Russia most in details professor Kinzerskij A.J., who is the author of two monographies devoted to this theme (2001, 2007) was engaged in questions of ultrasonic diagnostics of backbone pathology.
Results of our work have allowed us to develop some new criteria of objective ultrasonic diagnostics lumbar hernias of intervertebral disks (Patents of inventions: RU 2368401, RU 2371096, and RU 2372849). The method of ultrasonic diagnostics involves noninvasive, full safety for patients and a split-hair accuracy. To researches applied ultrasonic scanner Aloka SSD-4000 ProSound PHD, the sector gauge of 2,5-6,0 MHz.
We do about 2000 ultrasonic researches of lumbar department of a backbone every year. Earlier we had been spent some researches for definition of sensitivity and specificity of author’s techniques of ultrasonic diagnostics of a pathology of a backbone rather «the gold standard of diagnostics» for diagnostics of hernias intervertebral disks - a method of a magnetic resonance imaging (MRI). Have been defined sensitivity - a share of positive results of the test in group (in population) sick patients and specificity - a share of negative results of the test in group of healthy patients, our method of relative this standard. Under our data, sensitivity of ultrasonography diagnostics of a dorsal hernias lumbar intervertebral disks, with application of the technologies developed in sanatorium “Barnaulskiy”, reaches 94,1 %, specificity - 98,2 % (Oleynikov A.A., 2009; Remnev A.G., Oleynikov A.A., Saldan I.P., 2010).
Thus, modern ways of ultrasonic diagnostics of pathology of lumbar department of a backbone allows to provide objective diagnostics dorsal hernias lumbar intervertebral disks, to establish exact localization of pathological process, to raise quality of research, and also to provide possibility of an establishment of the control of results of treatment.
