METHODOLOGICAL APPROACH TO PHARMACEUTICAL SUPPORT IN PATIENTS WITH OCCUPATIONAL DISEASES AND METABOLIC SYNDROME |

Pharmaceutical support in case of occupational diseases is a system of medical, informational and organizational-methodological provision of pharmacotherapy implemented in various public health sectors and general pharmaceutical practice. In order to optimize pharmaceutical help programs for patients with occupational lung diseases and polypathology we made a medical social portrait of a person with chronic dust-caused lung pathology.
Average age of patients with chronic dust-caused diseases in in-patient care in the clinic of Hygiene Research Institute of Novosibirsk was 59,3±2,3 years old. 9,8% of them had higher education, 67,6% high vocational education. In 88,3% of cases their families consisted of 2 to 4 persons. 40,8% were alcohol addicted, 19% - nicotine addicted. 21,8% were registered as disabled of the second and third degree of disability.
36,5% of the research participants took at least one sanatorium and spa treatment. Total length of work experience made up 29,3±2,7 years, work experience with job hazards – 19,98±1,3 years. All the patients had associated diseases including cardiovascular system diseases (90,4%), gastrointestinal tract diseases (46,34%), bronchopulmonary system diseases (9,8%).100% of the patients under study suffered from locomotor system diseases, 21% of them had indocrine disorders including metabolic syndrome. 22% suffered from associated occupational diseases.
We analyzed range of medicines used during inpatient treatment. This analysis allowed us to reveal the medicines which were used to treat both occupational and associated diseases.
The most frequently prescribed medicines are vitamin C (85,1%), ambroxol (79,57%), methionine (76,19%), combination of retinol and α-tocoferol (64,29%). Vitamins were prescribed to the patients at least in 9,52% cases(vitamin B6), at most in 85,1% cases (vitamin B1). Vitamin E and glutamic acid were prescribed in 35,71% cases.
Mucolytic agents were prescribed quite often, almost in 100% cases, mostly ambroxol and antitussive drugs (in 79,57% and 45,24% cases correspondingly).
Bronchial spasmolytics were prescribed seldomer: at least in 2,38% cases (serevent), at most in 11,9% cases (berodual).
Drugs to cure cardiovascular pathology were prescribed at least in 2,38% cases (nitrosorbidum dinitrate, atenolol, ednit, nitroglycerine), at most in 28,57% cases (enap), 11,9% (cordaflex), 7,14% (egiloc).
Analysis of frequency of drug prescription in case of chronic occupational diseases at the stage of inpatient care shows that the following drugs are prescribed most frequently: ambroxol (12,95%), vitamin C (11,32%), combination of retinol and α-tocoferol (10,01%), methionine (10,06%). Vitamin E, antitussive drugs, enalapril, diclofenak, berodual, analgin, benadryl enjoyed over 2% frequency of prescription. The rest medicines including the ones for pancreatic diabetes and metabolic syndrome are seldom prescribed – under 1% cases.
The revealed peculiarities of the given group of patients will allow to adjust pharmaceutical help program in the way of rendering specific pharmaceutical services, selling over-the-counter drugs and optimizing range of drugs prescribed by doctors.
